Planning dates, how do you do it?

Honestly, I am finding it difficult. Want to know how you plan your dates. Do you discuss actual dates before you set in stone what you are doing with a partner? What about your vacation time? Just some insight on how others deal with their partners planning and even your own. I am trying to be a better planner and it's not easy!
Edit: For example, do you make plans and then tell your other partners? Or do you talk to all your partners then schedule? Very complicated.
