Looking to get healthier at 39

I'm 39, 5'9", ~165 lbs, and pretty out of shape. I think I eat relatively healthy (one egg, oatmeal with fruit, and a piece of toast with butter and jelly for breakfast; chicken breast, black beans, avocado, spinach and kale for lunch; dinner fluctuates between salmon/chicken/steak with leafy greens and/or brown rice; delivery once a week, usually Jersey Mike's; for dessert I make chocolate peanut butter protein balls with fruit). I don't do any physical activity, though. I'm tired all the time, I am laughably weak, I have pathetic stamina, and generally feel bad about myself. I used to try working out at home, but I need to get outside of my comfort zone if I'm actually going to make progress. I'm not looking to get buff (although , I wouldn't mind some muscle). I just want to be healthy and do what is best for my mind and body.

I would like to start going to the gym, but I'm very intimidated by the idea. I haven't been to a gym in years, and I wouldn't even know where to start. I have a heart condition, so I have to be careful with cardio, but I'm fine as long as I pace myself.

Any tips, recommendations, or encouragements would be appreciated.
