How do I get my doctor to listen?

Alright. So I need to know how to best get diagnosed.

I have been extremely hypermobile ever since I was a child, got called double jointed and stiff by my doctor as a kid. I remember bruising easily, and I dislocated my hip a lot as a teen. Now it's my wrists. My joints are very weak. My knees make terrible noises when I bend them and my wrists aren't very strong. I suffer all day from pain, from the moment I wake up to the moment I go to sleep. Im just so afraid I won't be taken seriously by my doctor because I'm on the heavy side, even though I was pencil thin as a child when the same symptoms presented themselves. How do I get my doctor to listen? (I have tried discussing this with her at first and she recommended physical therapy, which didn't help) should I also mention I have a higher heart rate while standing. (I know POTS and Ehler Danlos tend to go hand in hand)
