Is safety a concern when restraining / tying your bottom?

I hope this is ok to put in this sub. I need advice not for my dynamic but as someone who is looking to start making bondage gear.

For all you bondage tops, riggers, and general restraint lovers... Are you ever concerned about the safety of your gear? I can see how the answer is easily yes when it comes to rope suspension as there are risks of falling, nerve damage, loss of circulation etc... But say you're just going to use cuffs and/or a harness on your bottom. Have you ever experienced a near-injury due to the product? Are there improvements you want to see in the bondage gear industry that would make your scenes feel safer, and therefore, give you more peace of mind?
