Cornstarch keeps clumping in pudding

I’ve been trying to make some different kinds of pudding lately and my cornstarch keeps clumping. I’m not sure why because I’ve tried adding it different ways and watching the temperatures on each batch.

This latest round I put in milk, warmed it for a little then put in sugar, salt and cocoa powder. I then was mixing that for awhile until it got to around 200 degrees (just under) and added my cornstarch slurry of water and cornstarch. I me ed that until it got thick and added chocolate chips.

I’m thinking I let the cornstarch slurry sit untouched for too long? I mixed it and let it sit for a bit.

I’ve also added cornstarch with sugar and cocoa powder before and it still clumps.

I’m not sure what I’m doing to cause this I feel like I’m following the directions I find online. Any help is appreciated!
