Making a dumb thing smart with z-wave

We have an on/off switch along with light indicators to show if it's open or closed.
It's a 5 wire setup and requires a DPDT switch to open and close the valve. 3 of the wires are then used to go to indicator lights to show when the valve is fully open or fully closed. It's powered by 9-24V DC.

I'm trying to figure out what zwave switches and devices I can use to be able to remotely Open and Close the motorized ball valve AND get feedback to show if it's open or closed. Thoughts? Questions? Ideas?
