How very big channels read the comments

A channel I'm on reads many of the comments and even replies on older videos. Fraser Cain sources questions for Q&A videos this way, too. I'm curious how this is done - clearly they have a way of seeing every comment that comes in (like /r/.../comments
on reddit), is that for everything, is it just for top-level comments?
