I see this brought up pretty frequently, but none of the solutions offered ever helped me. I went through 3 controllers (one from each generation), upgraded from regular Xbox to x, updated controller firmware, moved router, etc. You name it I tried it. Anyways, a few months ago I saw a random post that it was caused by cell phone interference. They said moving your phone a few feet away would solve the problem. So I tried just moving my phone an arms length away... Sure enough the random disconnects stopped. It's been 3-4 months now that I've been doing this and I haven't lost connection once.
tl;dr it's your cell phone. Place it an arms length away and you'll stop randomly losing connection
