I'm trying to find all instances of DLC getting a standalone release on a disk for the 360. Not as part of a "Complete Edition", just on its own without the game. Here's what I have found to be available so far:

• Fallout 3 - The Pitt and Operation Anchorage
  
• Fallout 3 - Broken Steel and Point Lookout
  
• Oblivion - Shivering Isles
  
• Borderlands - Double Game Add-On Pack (Zombie Island of Dr. Ned & Mad Moxxi's Underdome Riot)
  
• Dragon Age: Origins - Awakening (Thanks to /u/bizzybarke, who informed me about this one)
  
• Final Fantasy XI - Wings of the Godess (thanks to /u/Karusa117 for letting me know about this one!)

There are also a few expansion packs released as standalone games which work without the original disk: (thanks to /u/SenorM3M3S and /u/realifesupernintendo for these!)

GTA IV - Episodes from Liberty City (Ballad of Gay Tony and The Lost and Damned)
Red Dead Redemption - Undead Nightmare
F.E.A.R - F.E.A.R Files (Extraction Point and Perseus Mandate)
