How do you approach writing combat?

My personal tastes lead to me reading a lot of stuff that involves combat in some way, from personal fist and knife fights to full scale battles. It's always interesting to see each author's varied approach to combat, from trying to describe a blow by blow action sequence to "zooming out" and watching a battle from a bird's eye view. For me,- "action" of this kind can be difficult to convey well in a text based medium like writing, so I'm curious:

How do you approach writing combat in your own work? What approaches do you feel work best when reading others, and what approaches do you dislike?
