Clarification of World Cup Rules Yellow/Red Cards

I am in need of Rule Clarification.

2 yellow cards = 1 red card and the yellow cards carry over from match to match not resetting until the quarterfinals.

So a yellow card game one, then nothing game two, then a yellow game three will result in a red card? Or just back-to-back games?

A red card means that player is off the pitch forcing your team to play a man down even if you have subs left. Also that player can not play in the next match but your team can still field a full team.

When the player returns is his slate clean, or is he a yellow away from another red?
