Well made weapon broken ?

Hi !
I repaired my sword myself, which shown then the symbol indicating that it would soon break (the symbol of a sword with the lower part in red). So it appeared with the adjective "Well made" after repair but the symbol in question did not change. It broke just afterwards, after taking only two hits. The thing is similar when I asked a blacksmith to do it.
Did I miss something or is it a bug? Thanks for your help
