How much to charge for my services?

I am working as a medical director for a specialized aesthetics clinic. I will be moving on to another position. I want to offer to stay on as a consultant and prescriber, but Iām not sure how much I should charge them? I take in 15k monthly but am there 40hrs a week.
When I move, I am considering asking for $4,000 a month to stay on remote as needed, for them to use my npi, license, to retain me as a consultant and DEA prescriber, as well as to sign an NDA. Am I lowballing myself?
