Farmers market find, propagating tips?

Hey all, bought this at a market about 2.5 years ago. I've never changed the potting medium, mostly because I don't know what species this is. One of the tips recently broke off, and after I put it in water it sprouted roots within two or three days. Photo here it's probably been in water for 2 weeks and I'm getting antsy about transplanting.

Aside from the ID, any recommendations for the new transplant soil/medium wise, as well as whether or not I should try and up-pot or renew the soil on the original? Thanks!
