E.G. an office is hiring rapidly because they need more people, and so "don't have time" to set up a formal onboarding process. So they ad-hoc train every employee without much pattern or continuity, and after a year they've trained fifty employees and had a dozen different people reinvent the wheel for what needed to be taught. So it ends up taking five or ten times longer than if they'd made time to set up a curriculum before the hiring spree.
