Here are the few things I can remember.

I distinctly remember a scene in a chapter where people in a 'Hell House' discover real bodies and 'demons'. This leads to the discovery of all of these tunnels where 'demons' have come up through. The tunnels connect pretty much the entire world.

In the sequel, the main character from before has been wandering the tunnels, and it has caused him to change. Flat, wide feet, night vision, bony protrusions from his head. The tunnels cause this to happen to people from the surface who stay in them.
