Devs who maintain websites after creation, how do you structure retainers?

I'm curious as to what industry standard is with retainers. You've created a website for someone and continue hosting it for them, but how much do you charge above and beyond what you're paying for hosting?
Additionally, do you include a certain amount of monthly/yearly service at no additional charge? I'm trying to figure out how to price and structure this for my own situation.
Lastly, what hosting do you guys use to have multiple clients' sites accessible under one login?
