In the beginning, Malkavians were the comic relief which as you can imagine tends to grow annoying when someone who is not funny thinks they're hilarious. Fortunately for those who grew sick of that style of play, awareness toward mental illness evolved so playing Malkavian had to evolve with it.
Well, now it's generally accepted that to play Malkavian correctly you want to at least try to play an accurate and inoffensive depiction of mental illness. Here in lies the problem with that. Mental illness is fought Inside the mind. This means that you're not going to notice anything wrong with that person until they do something out of the norm.
Well, most people who have mental illness try and keep their tics private. This means not only is their internal struggle "INTERNAL" but their outward tics are kept out of the public eye 99% of the time. So in actuality, you could go an entire session with your Malkaivan seeming like a completely normal person. Which defeats the whole point of playing Malkaivan.

The Malkavians in my game are quite normal to interact with. They might be eccentric, obsessed with an idea, or have milder symptoms of something particular but are otherwise not always displaying some mental affliction.
We play it that it becomes openly apparent on messy crits or bestial failures added the other effects that those rolls inflict.
Malkavian 1: obsessed with communal living and wanting to spread the word of its benefits to mortals and vampires alike. On dramatic rolls he disassociates and feels isolated and alone, even among others. Semi-Catatonic even.
Malkavian 2: has an obsession with the study of blood. Discovering secrets about it. Amplifying it’s properties. When he has a dramatic dice roll he fixates on analysis or imagines they are making an inspired breakthrough.
Malkavian 3: she was a classic dance instructor and choreographer fixated on perfection of her students. In her mild states she moves around gracefully and muttering “and… 1, 2, 3”. But when the dice speak she MUST dance or command other to dance and then she goes into instructor mode and gets emotional and upset when things aren’t coming together.
