Spayed cat smells like urine

We have a rescued female, spayed, who once a month will come up from the basement smelling like strong urine. The entire house will smell of it as well. What might be causing this, and what course of action would be appropriate if there is one. We also have one male cat but he doesn’t do this and is neutered/de-sexed.
Edit: we took her to the vet for this last month and they found nothing. This behavior is also aligning with my partners ovarian cycle.
