Healthcare Question for Europeans - are you insured? how?

how are you insured? how much do you pay for it?
if i leave my homecountry germany - i could opt out of healthcare - but then i wouldnt be insured. i think thats bad... there is some kind of travel health care that costs about 90€ per month - but i think its the best way to go.
do you have some alternatives? i heard there are some countries where healthcare is free? but i think that would only apply if i legaly live there? not just travelling?
