Either the russians blew it up, or the russians were stupid enough to allow it to collapse under the weight of too much water. It would be wrong to make any official declaration until they're sure which one it was.
The spillway damage was secondary, the dam itself did nearly fail.

No this is just modern British English, for example there are many examples in reports of car crashes where the wording "the vehicle left the road" is used. It avoids implying any cause while the cause is still being investigated.
These intelligence reports are obviously held to some quite strict editorial rules, most obvious is the use of "One Way Attack Uncrewed Aerial Vehicle" rather than "suicide drone" (and indeed instead of "unmanned").
