Stuff like this has happened in the past. I commonly affectionately call my friends "good boy" or "good girl." For example I have a friend who was/is on a weight loss journey and when not long ago he was telling me his diet. I helped him with his diet because I maintain a calorie count and track my workouts as well. He told me his meal plan he followed and I said "good boy."
One of my coworkers is a younger more entry level worker. I won't get into detail on my job but I am salaried and higher level and he is hourly.
He isn't my assistant or anything but I asked him if he could do something for me and he said, "Of course Mrs. (My Last Name)." And I touched the side of his arm and said, "good boy." I didn't think of it and just kind of did it and carried on. I was already heading back to my office and he was kind of still and not moving. I apologized and he said it was okay.
