Does anybody know of good guided tour companies in Germany?

My husband and I want to see some of Germany next spring or summer. I've been googling for small-group guided tours and looked at everything that Google brought up but haven't found what we want. Hope someone here can make recommendations for tour companies. Here's our criteria:

We want at least 2 days in these cities: Berlin, Munich

Perhaps also a day or two in Baden Baden or the Bavarian Alps area

Ideally a small group but if that's not available then a private tour is fine.

We're from the USA so we need an English speaking guide.

Guided the whole way, since even though we'll do some language learning before the trip, we're not entirely comfortable with the language barrier to get around on our own. This seems to be the part my Google search results fall down on.... the companies I found don't seem to want to put the guides on the train with us, or drive us in a motor coach between the cities.
