Any strategies of transitioning in a non tolerant country?

Hi 27yo pre everything here. I live in a third world country in Mauritius and its not tolerated to be gay here let alone trans. If i come out my family are definetely kickinh me out. I work in IT and can sustain myself independently, but i think my only option is to move to another country, any suggestions and how to make a move as it sounds scary and all the visa process looks complicated
