Hi everyone, I'm trying to purchase a ticket for a bachata festival taking place in Rome this year, but the website is only in Italian that I can see, and there are a couple of the fields I don't understand what it's asking me to fill in. This is the site:

From google translate, it seems that they all revolve around birth information, but I have no idea what they need. For the last one there's a drop down menu with country names in it, so I assume that means country of birth, but the two above are baffling me completely. Any help with this would be very much appreciated. Thank you!
Thanks for this - so it definitely refers to birth, rather than where I live? I live in London, but was born in Glasgow in Scotland so should I put:
Nazione: United Kingdom
Commune: Glasgow
Provincia: Glasgow
Glasgow doesn't really have a municipality or province that I'm aware of, it's just Glasgow City.

EDIT: I think there are what you would call provinces in Scotland, but we call them regions. But in 1994 places like Glasgow were just changed to City of Glasgow, and no longer part of the overall region.
