NHS Record: Removing dead name / incorrect gender references

i Have changed my title and name at the GP, but not currently planning to change my sex marker, complicates things more than I’d like right now.

Having said that, do I have any entitlement to request any references to my dead name or the ‘he’ pronoun are redacted? Or am I only entitled to that if I get a new NHS Number via the gender reassignment protocol?

Another question, how do I minimise risk of dead naming? I’ve contacted the 2 hospital trusts I’ve interacted with before to make sure they have the correct name and title, I’ve also had the change processed by the GP and sent to the NHS spine.
