A couple of weeks ago I found out that I was never reffered to the GIC despite being told my my mh nurse that she had done it over a year ago

Following your advice I have asked them to go through all my notes to try find proof of my referral to try persuade the GIC to backdate my referral

Unfortunately my old mh nurse did not document it properly and has since deleted all her emails and documents so there is no dated record of the referral

Following this I went to my gp asking them to refer me. They informed me that they have changed their practice policy to no longer refer to the GIC and that I should self refer

I have tried to self refer but there are bits on the form that I can’t fill out as I just don’t know the answer such as my gp records

So I have spoken to my new mh team asking them to refer me

Today I have been informed that they have sent the form off to the gp so they can fill it out despite me explaining that the gp will not do it

The bits of the form they did fill out (personal details) they didn’t even do correctly

Does anyone have any advice on what to do? I feel so lost, gp won’t refer, my team seems to not be able to refer, I can’t fill the form out fully myself It really feels like no one cares and no one wants to help :/

Has anyone filled out the form themselves and know how to fill out the medically bits?
