Platform(s): Probably PC since I have very minimal gaming on consoles but it could have been on some console that uses a screen as big as a PC monitor
Genre: Rail shooter-esq. Think Need for speed driving but your "car" can shoot and even jump
Estimated year of release: I know I played it before 2009
Graphics/art style: 3D-esq, what would be considered as bad cgi in today's age but the art was pretty decent for its time.
Notable gameplay mechanics:
You controlled a "car" (Car is just a placeholder term, maybe it was a ship?) You could drive around a small map that was surrounded by a fence/walls etc. Getting over the fence was possible but driving around the outskirts of the map was pointless as you would drive indefinitely to no destination.
Various power ups existed over the map, the only two I remember is a boost that gave your ship great speed for a few seconds and the other one boosted your car in the air extremely high for a few seconds. Not sure what the point of that was though.
The objective of each level was to collect 4(?) "shiny orbs" which were often either swarmed by strange looking enemies or in a puzzle. The orbs would unlock the exit to the next level
The first level was like a small carnival and the one of the orbs was in the corner with a lot of strange things on stilts guarding it.
Another type of enemy looked like a strange burger-shaped thing which wouldn't actually try to attack you and if you bumped into it, it'd be pushed back. Shooting it would make it fly up into the air and there was a spaceship on that level that dropped these burger creatures and had one of the orbs, shooting the burgers to blow up the ship was how to get that orb. That level was a kind of desert, one orb was stuck in a sandstorm tornado. Never figured out how to get it.
Another level had an orb being flung around by catapults, never got that far.
Other details: This is the "Opening cutscene"
You basically have a first person view, you're looking up at the ceiling of a hospital being rushed to emergency you can see a drip in view but otherwise the character is only looking up at ceiling with typical emergency hospital ambiance
Eventually put on a wheel chair and taken into an elevator, you can't see who puts you there but you can see staff running around the hospital just as the door closes
The elevator continues to go down and at first you see normal floors and clean rooms etc
But the lift keeps going down and down and eventually going past even the basement. The floors you can see through the small window on the elevator door are becoming more decrepit and rusty
You eventually make it the bottom floor, in some abandoned area of the hospital, your wheelchair is pushed slowly through the floor. The character scans left and right looking into different rooms all with no door
Each patient there has some strange machine next to their bed, you can only see the patient lying down and their feet which obscures the rest of their body because you're looking on from a wheelchair so you don't have a lot of elevation
But you can see that the strange machine has a metallic arm that goes right over their face and covers their eyes
Everyone is screaming
You're strolled down the hallway on this abnormal basement floor and eventually your wheel chair is taken to the last room at the end of the hallway
You're turned around so you can see the hallway again, and some weird looking device comes into the front of your view. It's shaped in a way that implies that it goes into your eyes And there are two very long and sharp spikes. Just two of them. They clamp onto your face, the spikes go into your eyes. The character immediately begins to scream and the game begins.
