Ok so to preface this I work in an industry with a lot of mechanical elements and different industrial processes, basically if I fuck up people get hurt, thankfully my fuck up was rather small and the emergency system caught it but not in time, now on to the story;
it’s a normal day at the plant and we are doing a batch of Hydrozine, it’s basically formic acid. Now we typically make it in giant vats but our main vat was down for maintenance… nobody tells me. I proceeded to start the process from the control room without double checking my valve lineup, now understand the second I press this button it dumps in a bunch of water into the tank… the main tank. So basically I press the button and the next thing I know I’m getting a call on the emergency phone… I have just flooded the plant as the service hatch was open. Thankfully most everything is waterproof and a few workers just got washed out of the service hatch not a big deal, anyway eventually the system starts to pressurize with co2 and the emergency trips “no pressure in main tank”, just to be clear I get the notification then the call. Anyway nobody was injured but I did get a talking to and the maintenance chief was torn a new one for not informing me.
TLDR; flooded a factory with a push of a button.
