How it is identified that ptc has spread to lymph nodes ?

People who had the situation where after their surgery was done, later on they found there is some cancer left in lymph nodes. Or may be 1 year after surgery you found out. How was it identified? Did you go for ultrasound of the side of neck region ? Or was it other test? If you dont any symptom how do you find out there is cancer in lymph nodes ?
