How does red key work?

I just unlocked red key and tried looking it up on a binding of isaac item website and I couldn't wrap my pea brain around it. I know you can get tainted characters with it at home as I've done so already, but how do you know where to use it? I saw a red secret room out of bounds a few runs ago but I Have no clue how I Would've gotten to it. Thanks for the help!
