Best way to install a brand new SSD + HDD in a caddy, while preserving the partitions?

I have a 1TB HDD installed currently, and i bought a new SSD of 500GB. And also bought a HDD caddy to replace my DVD rom with.

I wanted to have the C partition (with Windows and softwares installed) transfer/cloned to the SSD. And the other partition D, has most of my personal files. Which i want to keep as is, in the HDD as it will be secondary drive to the primary SSD.

Could you suggest me the process of doing so, as well as any freeware/open source softwares that could be useful.
