Can't format SD card with Raspbian OS -- tried on both Windows and Manjaro

In Windows, diskpart says "Windows failed to..." reformat the partitions even after disabling read-only attribute, while in Manjaro, fdisk just pretends that it did something successfully, telling me that it needs to reboot and then sync. When I do reboot, there are no changes.

All I'm trying to do is flash a new Raspbian OS, but Rufus also just pretends that it wrote the image file into the SD card successfully, but all my old files and the user/pass are still there. I have no idea what is going on.

I am using a USB-to-SD-reader, if that matters at all.
