Someone is hacking me..

Hello,

SO I have been noticing some weird stuff going on lately. Icons moved around, their names edited to weird things and stuff like that. I know that someone is somehow messing with my stuff. Here are some facts.

-No one else has access to my computer in my household, it is definitely from an outside source.

-When I started playing Diablo 3, I was hacked twice, the situation was resolved with a rollback of my account and the installment of an authenticator from Blizzard.

-I believe there is a possible "keylogger" as well on my pc, because even though I changed passwords prior to getting my authenticator, I was still hacked repeatedly.

I currently use a free version of Avast antivirus software and Malwarebytes anti malware. How can I find the source of this hacking on my pc? I mean somebody is just freelancing around my computer accessing my information, and I am very worried. If anyone has any tips or strategies please let me know. This guy is mocking me.
