Early 401k Penalties

Years ago I withdrew my 401k and closed my account with a previous employer. I have never paid the taxes or received a bill from the IRS. I contined to file my taxes as I normally would since then. Is there a law after a certain amount of years? Would I be accumulated debt on this tax? Looking for any information available as I wouldn't want this to bite me in the ass in a decade from now. Thanks.
