Understanding what happened to A. Metallica

So around two months ago I bought a sub adult A. Metallica from a reptile expo. I have a lot of years of keeping tarantulas and never had one act like this, he was fine and eating when originally bought but had a very small abdomen with a lot of hair missing. I setup a proper dark cage for him with a water dish, lots of places to hide, fake plants, etc. But after about a week, he refused food and was very lathargic. He did this for around a month. My first guess was dehydration so I setup a ICU and he seemed to be getting better, he was drinking off the paper towel but still refused to eat. After a few days in his ICU he was doing much better so I proceeded to put him back in his cage. Today I go to check on him (only two days since I removed him from his ICU) and he unfortunately has crawled behind one of his logs and passed away. I've kept many avics and know about SADS but the way he seemed to be okay then mysteriously die has really confused me. No prey was left in cage, he had a water dish, and for some reason somehow one of his legs and pedipalps became detached and was at the other end of the cage away from him. I picked him up and he has a large hole off to the side of his fangs as if something ate him or ate their way out. I'm not sure what happened to him but I am concerned he might of possibly had a parasitic infection which could spread to the rest of my collection. Does this sound like that? In all my years I've never had a parasite infected tarantula so any input would be great. TL;DR : Avic seemed okay in ICU, died in cage, has large hole on underside and missing legs. What happened
