M40 here. I've been interested in exploring male multiple orgasm and tactric practices for a while now. I used to be multi-orgasmic for about a year or two before the full onset of my puberty, and I'd like to get back to those sensations. I remember the feeling of climbing gradually incrementing plateaus of pleasure, and the orgasmic sensations radiating out from my core in waves--all very different from the ejaculatory orgasms I've had for the better part of 30-odd years after.
However, for the past 10-15 years, I've been dealing with intermittent pelvic floor tightness. This went undiagnosed for the longest time, even by myself. Every so often, ejaculations would just be painful, or pain would radiate out into (the underside of) my glans. I went to a urologist, wondering if I had an STI or maybe an inflamed prostate. It seems now that it's my pelvic floor, as also evidenced by occasional spasms and cramping, both in a sexual and non-sexual context.
I haven't been able to pinpoint the exact trigger at all, can't find any rhyme or reason to it. It seems to happen more frequently with masturbation than in partnered sex, and when it happened in partnered sex, it was sometimes after edging or with a very full bladder.
I've been to a physiotherapist who used biofeedback therapy; basically inserted an electrode and made me contract and release. She said she'd never seen a pattern like mine before: while it was clearly elevated, the peaks seemed to follow a pattern, which she thought was perhaps my breathing. After about 2-3 sessions I had an odd, sudden, dramatic improvement in relaxation, and after about 5-6 sessions total, she said my readings were "as good as any person could wish for".
The painful ejaculations that radiate pain into my glans have since all but disappeared, but I still get perineal and rectal cramping, especially at times when my pleasure becomes more intensified.
All this to say: I know a lot of stress is put on practicing kegels; both long holds and in rapid succession.
I worry that this will be detrimental for my pelvic floor tightness, and was curious if anyone (m/f/x) had any experience combining tantric sexual practice and pelvic floor tightness.
