New software change at the company, but need to get end user feedback from the staff working from home beforehand

Hello there folks,

We're rolling out some new software where I work to around 500 users that are working from home.

There will be a few weeks beforehand where portions of staff are rotated to use the software, and leave any feedback before we go live. Once fully live, the old environment will be gone - so their input on averting potential mistakes is pretty crucial.

What is the best way to get a comment card/questionnaire/survey in front of the users, and back from them?

I'd like to create the questions, and have them be able to type up responses to some questions, and maybe like a "bubble" fill in rating on some other questions.

How is the most ideal way to deliver something like this to the staff - and get feedback received that can be pretty easily digested and analyzed?
