How do you think his when played ability works?

A friend and I were debating if the unit would have to have damage on it which is healed before the shield comes out or if full health unit would still be able to qualify for getting the shields?

Exactly this. “All” can equal zero.
If it wanted to require damage as a condition to the shield, it would have said something like “if you healed at least one damage this way, give 2 shield tokens”
