I'm a t10 incomplete,Asia level c, I barely have a primary care dr. I was let out of the hospital 3 years ago in April . I was in for 2 months and let go with zero information, no social worker, no advocacy, really nothing more than a packet telling me when to take what mEd's. My family is incapable of helping to care for me. I'm in Minnesota. I've been homeless since my injury. I get long term disability through the union of 1500 a month. I am fairly self sufficient NOW . But Its been a struggle to get here. I don't know who to contact and the places I have contacted seem to offer no help at all. I was denied cadi waiver because "homelessness was my real problem" and when I dealt with that I should reapply. I just came up for housing voucher and hopefully will be finding an apartment soon. I feel like I've been Set up to fail and would like to know if there is any way to hold the hospital or social worker there accountable. Not to mention any resources that could help me now.
