How common is fentanyl laced marijuana really?

I work in Child Welfare and I have had several clients who tested positive for fentanyl saying that it is due to smoking marijuana that must have been laced, however it often seems from history or witness statements that they were using fentanyl intentionally. I wonder if anyone’s experienced cases of accidental fentanyl exposure through marijuana they felt to be true!
