No credit for achievements?

I haven't been getting credit in Steam for my achievements. I just played a game where I killed Donu with [[Feed]] and didn't get credit for it. I also haven't gotten credit for killing The Transient, having 25 focus, having 999 block or completing the ending with the watcher despite doing all of them. Am I doing something wrong?
