Alchemy health potion trouble

So I have an alchemy farm going and I can't figure out how to mke a completely restore health potion. I have max skills to 100 and I'm using ingredients: swamp fungal pod, blisterwort, and blue mountain flower. Am I missing an ingredient or something?
