WTH Happened here? Help needed.

Day 2, Day 4, Day 7, and Day 14.

So as you can see, things looked beautiful for the first ten days. Then bam! A spot of mold. Which took over the entire box in two days.

Any ideas as to the cause? I did stir it up at one point bc the grain was clumped together.

The media was covered in mycelium so I figured it was already vigorous enough to leave no room for mold spores.

I have another bag of grain ready to go. What should I do next time?
