Forms and State Service fun

So inherited a farm, IP forms throw famous State Service config error. Rebuilt service from powershell, still f'd. Thoughts?
Just to confirm:
-deleted svc
-deleted db
-created svc app
-created db and associated
-associated proxy
-confirmed svc and proxy display as started in CA
-confirmed svc was assocaited with web app
