No ejaculation happening and it bothers me

I am a m38, and I have struggled with not ejaculating. I had surgery on my urethra as a small child due to a birth defect.
As strange as it sounds, it took me years to understand what other guys mean when they spoke about their orgasm. My sperm is kind of stuck mid penis and dribbles out when I get soft.
I am embarrassed by it with sexual partners. I don’t feel manly.
How would you react, and what do you think I should do?
