My grades are shit

Im in 8th grade, and I have 2 days till my winter break starts. I'm failing math and science, and I'm going to get a retention notice if I don't bring my grades up. At this rate, I might end up just becoming homeless. Any help?

I had bad grades because I was constantly trying to off myself and I couldn't remember anything after it was out of sight. I would write in my planner then forget to look at it, do the assignment then forget it at home, flip signs for no reason in math class with no memory of doing it.
Turns out I was just fresh out of norepinepherine and had to buy some at the store to make my brain work properly.
But everyone kept telling me it was my "work ethic". If I just tried I could be so smart. I was trying.
