Asking for guidance on what possible issue

Have had this model fail three times in the same way. Think the first time was forgot to put supports in the hollow portion of the model. Adding more supports has led to the same results. Thought it was due to my inexperience to manually supporting models but some of these ribbons seem to be coming off of the support braces themselves?
