MY FIL recently had surgery for esophagus cancer. This is what we have been dealing with:
July 10 - surgery
July 12 - moved from Pepin ICU to Cardiac floor room xxx, sign stating Strict NPO on patient's door
July 13am - Staff brings patient glass of water, patient and patient's wife inform staff patient is under strict NPO orders
July 13 afternoon - incident caused Doctor to reprimand nurses, places sign above patient's bed stating Strict NPO- NO ICE CHIPS - DO NOT REMOVE NG TUBE
July 13 8pm - Staff disconnected drainage tube, spilled hazardous fluids all over phone, desk.
July 13 8:30 PM - Patients family clean spilled bile after staff neglects to do so.
July 14am - Reported aggression and delirium due to medication to blonde duty RN
July 14 5pm- Patient's NG tube is removed, allegedly by patient, while patient's family is absent for 1 hour.
July 14 6pm- Patient's daughter asks duty RN why patient's NG tube has been removed, nurse is rude and tells daughter patient removed tube
July 17 9am- Patient is taken to radiology for Barium Esophagram, a test that will determine if patient is able to drink and eat.
July 17 11am- Patients family inquires about test, is informed Barium Esophagram has been performed and they are waiting for results.
July 17 3pm -Patient's family is informed that test was never performed, but will be performed shortly.
July 17 3:30pm - Patient's family asks for more mouthwash to swab patient's dry mouth, is given water instead because they are out of mouthwash, receives mouthwash an hour later.
July 17 4:30pm - Patient is taken to radiology for Barium Esophagram.
July 17 5pm- Patient's family is informed by radiology personnel that Floor staff were supposed to hold feeding 12 hours prior to test, and thus they are unable to perform test.
July 17 5:30pm- Patient's daughter asks duty RN on unit why father's feeding tube was not removed prior to surgery as required, staff becomes hostile, argumentative, and dismissive with daughter.
July 17 6pm - Daughter requests to speak with nursing supervisor. Supervisor is polite, but informs patient's daughter that staff is not knowledgeable with regards to patient's surgery or the procedure for Barium Esophagrams, Says that test will be performed the next morning.
July 17 11:30pm -patient's IV and feeding tube are removed by staff
July 18 10:30am - Patient's barium esophagram is performed.
July 18 12pm - Patient's wife notices IV is not connected, asks duty nurse if she can reconnect IV, Duty nurse informs her that she cannot connect IV unless ordered by doctor.
July 18 2pm - Physical Therapist comes for Patient's walk. Physical therapist becomes visibly annoyed and rude when informed by patient and his wife that he is too weak to walk due to being dehydrated.
July 18 6pm - Patient's daughter called and speaks to nurse via phone, nurse informs her she cannot reinstate the IV without doctor's orders.
July 19 10am - Doctor notices patient's fluid IV was never reinstated after it was removed (despite patient's family's requests), notes patient is dehydrated.
July 19- 12:30pm - Duty RN informs patient's wife that patient is dehydrated, patient's IV is reinstated, after over 36 hours disconnected. Water is also added to his feeding tube to help with severe dehydration.
-Several staff members have been consistently rude and dismissive to patient's family. (Exceptions are A. and M.)
-Patient waits a minimum of 30 minutes after pressing emergency call button, observed several times by patient's family.
-Patient's room is not cleaned daily.
-Patient's clothing is not changed when soiled during the day.
-Have observed staff members being rough with patient when transporting
-Patient's family feels obligated to stay with patient in shifts 24/7 to make sure he is getting proper care.
