So I came across a man hanging in a tree and I shot him down at the river just as I crossed in to Beaver Hollow from Roanoke Ridge on the road. In his pockets, of course I looted him, he had a note referring to being captured by some people and escaping a cave. Immediately, I began searching.
I came across one just off the path that is located directly where the "A" in Annesburg is on the map. Just under where it says Beaver Hollow. I didn't see any mutilated bodies out front nor inside and the cave ended up being completely unpopulated.
It was filled with different types of crates/barrels and wagons that looked stolen. There were very few heads on spikes but really, no gore or blood at all. At the end of the cave had been set up to be lived in. I found two sleeping bags and a chest with some jewelry and coins inside but also a pile of bones which I believed to be a cow, no blood. I also found an exit at the back of the cave where you ascend in a spiral through a series of ladders/ledges and inclines until you appear from the top of the small hill that makes up Beaver Hollow.
The whole thing was very well lit and had burning fires so I believe there's just some parameters I haven't met? Am I supposed to come back at a certain time? I would say I found the cave entrance at about 9:00 p.m. in game and exited at about 9:45 p.m.
I'm also fairly late game but I forget the exact chapter. Arthur isn't feeling too hot and just went on a hot air balloon ride with Mrs Adler to see their friend on an island
